Current studies show that Peyronie's disease (plastic hardening of the penis) is an inflammatory disease of the penis where the elastic layer (tunica albuginea) of the penis in the central tissue (corpora cavernosa) is damaged. Consequently, the affected area loses elasticity and, when filled with blood, does not stretch but causes the penis to curve towards the scar. Peyronie's disease is the most common cause of excessive penile curvature.

The process begins with the oxidation of various fats and protein molecules, leading to inflammation of the elastic layer (tunica albuginea) and the corpus cavernosum. This continues until the collagen fibers are finally replaced by non-elastic fibers that form the fibrous plaque (hardened nodules) so characteristic of this disease.

These plaques can be felt under the skin, usually on the top or sometimes on the sides of the penis. Most of them dissolve naturally, but sometimes they persist and, over time, cause:

Abnormal curvature of the penis (usually upwards);

• Erectile dysfunction and/or pain during erection;
• Shortening, loss of thickness and/or malformation of the penis (cuts, necks, hourglass shape);
• Sexual problems caused by anxiety and psychological issues.

Currently available treatments are not yet completely satisfactory and, for the most part, only help to alleviate the symptoms of Peyronie's disease. For less complicated cases and in the early stages, there are numerous medications and compounds (mostly anti-inflammatory and anti-sclerosing) that are used and tested.

If penile curvature persists into the more advanced stages of the disease, when the pain has already decreased, surgery is necessary to correct it.

However, this treatment is very invasive and can sometimes result in penile shortening that can reach 4 cm (1.6 inches).
